Standard Subcontract and Subcontract Addendum for Public Works Projects.    For your convenience, the below forms are provided in the following formats: Ralph Larsen & Son, Inc. Standard Subcontract  (word) Subcontract Addendum for Public Works Projects (pdf) In addition the following Forms and Schedules will be sent with the Subcontract for completion and returned.  Subcontractor Detail Sheet (word) Request for Material Safety Data Sheets (word) List of Potentially Hazardous Substances Commonly Found on Construction Projects (pdf) Schedule “D” – Affidavit of Subcontractors (word) Schedule “F” – Request for Taxpayer Identification Number Form W-9 (pdf) Certified Payroll Reporting (pdf) You will also receive a blank RFI form, a map of the project site and all preliminary lien information.  Please keep these for your use.
